Meeting Patients Wherever They Are
Every patient’s circumstances are unique, a fact reflected in our individualized plans of care. Our teams provide services in a variety of settings:
- At home
- In an independent living complex, assisted living residence or skilled nursing facility

- In the hospital
- Or at one of our Hospice Houses
Two Treasure Coast Hospice Houses, one in Martin County and one in St. Lucie County, offer round-the-clock, hospital-level care in a home-like setting.
Care At Home
Most people receive hospice care in their own home. Our team of dedicated, specially-trained professionals provides compassionate support from every perspective.
Levels of Care
Our teams are trained to complement and supplement care supplied by staff members at skilled nursing facilities, assisted living residences or other supported living settings. We work to offer services that can improve quality of life and provide comfort and support.
Treasure Coast Hospice Comfort Care
- Our team members visit patients at home or in a long-term care or assisted living residence.
- Your primary nurse will collaborate with you to decide how often to schedule visits.
- Day-to-day care is provided by family members or other caregivers.
Crisis Care
- This option is available when a patient requires extra care for a short period of time.
- The goal is to keep patients in their homes or other supported living settings.
Hospital/Inpatient Care
- Inpatient care is offered at a Treasure Coast Hospice House, a hospital or an approved nursing home.
- It is available when symptoms are too difficult to manage at home.
Respite Care/Caregiver Relief
- Treasure Coast Hospice steps in when a caregiver is temporarily overwhelmed or exhausted.
- In this situation, a patient receives care at an approved nursing home.
- Medicare limits coverage for respite care to five days maximum.
